Frequently Asked Questions
Signature membership requires a 6-month minimum commitment.
Corrective membership requires a 12-month minimum commitment.
Early cancellation prior to the minimum commitment may result in a cancellation fee. Written notice is required once your minimum term is complete.
Memberships are non-transferable and valid only for the enrolled member.
Membership benefits may not be combined with other promotions unless otherwise stated.
Botox units included quarterly for Corrective members only, are non-transferable, and subject to provider approval. Units have no cash value and may not be exchanged.
